Continuing Healthcare
If your main need for care is because of your health, you may be eligible for NHS Continuing Healthcare (CHC) funded support.
This is a package of care for adults 18 or over, which is arranged and funded by the NHS. The care can take place in settings outside hospital, such as a care home or your own home.
Your eligibility for CHC will normally be assessed by a health professional or social worker as part of your ongoing care. If this has not happened and you think you may be eligible, contact your allocated social worker.
